Payment Information

Payment Options:

1) Make checks payable to Mid-America Reformed Seminary and mail to the Seminary OR pay with your check on the morning of the golf outing at the golf course.

2) Pay online with your credit card through our online form (which we will link to after this registration form is filled out).

$130 = 1 golfer (golf, cart, range balls & dinner)

$520 = 4 golfers (golf, carts, range balls & dinner)

$100 = Hole Sponsor

$500 = Event Sponsor
